Limp Bizkit founding member Sam Rivers died in own bathroom after reportedly falling off the toilet.
Rivers, 48, was found dead in his Florida home back on Oct. 18.
TMZ reports the incident report makes it appear as though Rivers fell off the toilet. Per TMZ, a caller named Keely said she found Rivers lying face down in a pool of blood in his bathroom.
She attempted CPR, per the report, so did a deputy upon arrival.
The deputy said Rivers’ face and neck were dark blue, per TMZ’s, and that the rest of his body was flush. The report noted a pulmonary embolism is usually the cause of such symptoms.
Keely, per TMZ, told officers that Rivers had a liver transplant in 2018 and was on medication.
The band first announced Rivers’ death on Instagram.
